Introduction
Explicit of the fourth book from the Chirurgumena of Heliodorus. Four papyrus fragments (fr. A: 2,2 x 1,9; fr. B: 1,6 x 3,1; fr. C: 2,5 x 6,7; fr D: 10,5 x 17,1 cm) from a volumen written on the recto along the fibres (the verso is blank); fr. A and C preserve the end of several lines and part of the intercolumnium; it is not sure if fr. A, B and C belong to the same column. Fr. D, which conserves upper (3 cm), lower (1,2 cm) and right (2,8 cm) margins, preserves a whole column of text of 23 lines (9 x 6,5 cm), followed in the lower margin by an underlined and supralined subscriptio specifying the name of the author, the title of the treatise, and the number of book with the monogram of ὑπόμνημα (Ἡλιοδώρου Χειρουργουμένων ὑπ Δ). The text contains some scribal corrections with text added supra lineam (fr. C l. 13; fr. D ll. 16 and 21; perhaps l. 9), three elisions (fr. D ll. 4 and 14) and some dieresis above ι and υ (fr. D ll. 4, 16, 17, 22). The content of fr. D deals with some indications of nosology and therapy in case of damage of sphincter. Due to several reasons (excision, erosion or paralysis, ll. 1-5), the injury causes an involuntary flux of faeces: if it is recent, it could be nurse with astringent cataplasms, otherwise the damage is enduring. The other fragments are so badly preserved that nothing can be said about the subject. The hand, very elegant and quite sloping to the right, is a formal mixed hand; the refinement of the handwriting and the wideness of margins could indicate that the volumen was copied for library keeping. According to paleography, the fragment could be dated to the beginning of the third century AD.

According to Manetti, maybe ]εν ἄγειν is better than ]ἐνάγειν, because the verb ἐνάγειν is not usual in medical technical language.

ἔσθ’ ὁτὲ is equivalent to ἔστιν ὅτε (Marganne); ἱδίαν with rough breathing is a typical characteristic of the koiné and it is well attested also in late prose.

ἀδιορθωσίας is an hapax, therefore the adjective ἀδιόρθωτος is well attested.
